Cancer of the oesophagus is a very poor prognosis disease and the adenocarcinoma subtype has increased 6-fold in the last 20 years. The cancer commonly evolves from a pre-cancerous condition called Barrett’s oesophagus providing opportunity to detect it at an early, curative stage prior to invasion. In this seminar I will describe our advances in understanding of the molecular evolution of Barrett’s oesophagus to oesophageal adenocarcinoma and how we can apply this knowledge to develop molecular diagnostic approaches. The most promising application of this research so far is the Cytosponge TFF3 test which is a practical approach to diagnosing Barrett’s oesophagus in the primary care setting and is now in late stage clinical trials.
