DA221X, DA222X, DA223X: Evaluation form

Before the presentation, the evaluation form should be filled in first by the student, then by the supervisor and sent to the examiner. The examiner completes the form after the presentation and sends it to the thesis coordinator by the examiner when sending the pass grade.
