IVIM Diffusion MRI analysis and biomarkers for chemotherapy response evaluation

Abstract: Intra-Voxel Incoherent Motion (IVIM) is special Diffusion MR Imaging protocol that is acquired at multi bvalues using multiple diffusion gradients. This is a unique protocol as it is sensitive to both free water diffusion in extravascular space as well to diffusion in intravascular space (pseudo-perfusion). Thus it can capture both diffusion and perfusion information of the tissue region at the same time with no external contrast agent. IVIM has been shown to be useful in clinical evaluation of various tissues such as breast cancer, prostate cancer and liver cancer. However, there are challenges associated with IVIM analysis that even after 30 years of its history, IVIM is still not found suitable for routine clinical use. This talk will discuss, concept of perfusion and diffusion MRI especially IVIM. Also brief on various models for IVIM analysis and discuss assumption and limitations of these models. It will be followed by our experience with IVIM in last 5 years. We have developed an IVIM analysis methodology using penalty function that has shown promising results in terms of data quality ( aapm.onlinelibrary.wiley.com/doi/abs/10.1002/mp.12520 ), in talk this model will be explained and share our findings in clinical applications for Osteosarcoma and prostate cancer. We have also explored IVIM analysis for biomarkers in response evaluation of chemotherapy, I will share our findings in this direction. Multiparametric MR image analysis is another technique that is widely used in MR community now, we have used multiparametric MR analysis for non-invasive assessment of necrosis in tumor as a marker for chemotherapy response; key finding of this study will be also presented.
